My mother lived at East Bay Post Acute for several years and made many friends there. Unfortunately, during that time I have seen a significant decline in both care and overall service. In the most recent state inspection in [date removed], the facility received a one star rating out of five, the lowest in the county. The facility is severely understaffed for the more than 90 residents who live there, relying on a state staffing waiver to operate. Management turnover is frequent, about twice a year, which contributes to ongoing instability. A previous administrator, [name removed], focused resources on cosmetic upgrades such as IKEA furniture and landscaping for a photo opportunity for a new website. Since then, however, the grounds have deteriorated. Picnic tables are often dirty and are even used to store Roundup herbicide. Resident accommodations are inadequate. The activity room can hold only about 16 residents, and posted menus are displayed so impossible to read.

The facility is severely understaffed for the more than 90 residents who live there, relying on a state staffing waiver to operate. Management turnover is frequent, about twice a year, which contributes to ongoing instability. A previous administrator, Conners, focused resources on cosmetic upgrades such as IKEA furniture and landscaping for a photo opportunity for a new website. Since then, however, the grounds have deteriorated. Picnic tables are often dirty and are even used to store Roundup herbicide. Resident accommodations are inadequate. The activity room can hold only about 16 residents, and posted menus are displayed in a way that makes them nearly impossible to read. Security is also insufficient: it is possible to access more than two thirds of the facility directly from the parking lot without passing the front desk. Although parking spaces are labeled “visitors only,” staff routinely park in them, especially in the lower lot. The physical condition of the building presents serious accessibility issues. The elevator has reportedly been out of service for years, forcing wheelchair dependent residents to exit from a rear door and travel down (and up!) a steep, poorly maintained, and potholed driveway. The telephone system is antiquated—there is no answering service, no room phones, and unanswered incoming calls simply ring and disconnect. The front desk itself is understaffed and frequently unmanned during mornings, lunch hours, and evenings. The building is more than 60 years old, and its infrastructure reflects that age. Electrical power in resident rooms is often limited to a single duplex outlet shared by multiple patients, daisy chained with adapters and power strips. Fire safety is also concerning: there are no evacuation maps in approximately two thirds of the building. The only visible evacuation map is for the second floor east wing and directs residents to stairways, which would be unusable for many disabled residents—especially given the staffing shortages. Physical therapy services are likewise understaffed. Therapists are working weekends in an attempt to meet resident needs, yet patient Medicare therapy days are often exhausted with minimal functional improvement. Finally, I was disturbed by an account shared by a friend involving the current administrator, Calvin. When my friend reported a lost personal item belonging to a resident family member, Calvin allegedly responded dismissively in a hallway, stating that he did not work for them and that he did not like them based solely on appearance. He went on to say that while staff liked the resident, nobody liked the family member. I would not recommend this facility to anybody. My 101 year old dad was abused and due to a nutritional and hydrating program far below standards he lost 21 pounds in three weeks and nobody even tried to reverse the weight loss. He had to be sent to hospital on April 18, 2026. The rehab staff is great headed by Priyanka. They all belong at much better facilities where their talents can shine instead they are overshadowed by the poor performance of others.

I am deeply grateful for the staffs kind and consistent communication with me during my 75 year old brother’s end stage of life. I was 3000 miles away, but they went the extra mile to help me feel as if I was right by his side. His goal was to be rehabilitated, unfortunately his liver damage wouldn’t allow this to happen. We spoke daily, this was his favorite place, the best food, the best teck guy to help him with his computer or phone, the best pt group and nurses. He said the pt ladies were gently encouraging and he attended every session for weeks until he couldn’t. And when he couldn’t even talk anymore, these kind, compassionate ladies would go to his room, call me on his phone and place it on his pillow by his ear so I could talk to him. Because he was on oxygen, they would make sure I could hear his breathing as he would hear me talking.

My experience here has been more than great if I could put it in the words I would say a miracle when I came in I couldn’t walk I couldn’t do anything by myself but after a month being here and getting myself back together I can walk and I feel 1 million percent better. The staff here is out of this world especially a young gentleman by the name of Connor who goes over and beyond his duties to help families reconnect in very warm and loving environment I would suggested this facility to anyone who needs the acute care they provide.

My name is Cathy Jenkins and my experience at this facility is amazing, and the staff and CNA’s are respectful, especially Erika, Garibay Princess , Udoka are very friendly and helpful CNA nurses as well as Vanessa Wu. They are awesome to communicate with. I love this facility and the other staff members.

The nurses there….they show a lot of compassion ..I’ll name a few nurses that are awesome Renee…Marcella…Andrea to name a few…but really they are all the best CNA’s by far .friendly staff they treated me like family…clean to facility has no bad smells .Thank You Post Accute Staff….& Rehab….

The facility has been overall excellent with some minor deficiencies but those have been attended to by the current administration that has had a very positive effect on the service here. I would recommend this facility for care of the patients.

Great service, response to the call lights is 9/10 staff knows how to take care of stuff. I like watching the birds in the patio and the maintenance guy was nice enough to get me a bird feeder for the patio window so I get to watch them up close. Staff has great attitude.
